PURPOSE OF THE BIOMETRIX™ BIOSCAN

The BioMetrix™ BioScan is a research-grade health profiling tool that uses advanced biochemical technology developed by Dr. Goodenowe Research, to evaluate individual health trends and biomarkers. This scan is provided for educational, informational, and research purposes only and is not intended for diagnosis, treatment, or cure of any medical condition.

 

ACKNOWLEDGEMENTS

I acknowledge and understand the following:

 

1. Non-Clinical Use: 

  • I understand that the BioMetrix™ BioScan is not a medical test and is not intended to diagnose, treat, or cure any disease or health condition.
  • This scan is a research-based health insight tool meant to support my personal health awareness and understanding.

 

2. No Provider-Patient Relationship: 

  • I acknowledge that I am a client, not a patient, of the BioMetrix™ BioScan provider.
  • The practitioner or consultant reviewing my results with me is not acting as MY licensed medical provider, but ACTING ONLY AS A health consultant.

 

CLIENT CONSENT

3. Consult Your Own Medical Provider:

  • I agree to consult my licensed medical provider for any health concerns, symptoms, or treatment decisions.
  • I understand that I should not alter or discontinue any medications or therapies based on the results of this scan without speaking to my healthcare provider. 

 

4. Voluntary Participation: 

  • I am voluntarily choosing to undergo the BioMetrix™ BioScan and understand its purpose and limitations. 

 

5. Use of Information: 

  • I authorize BioMetrix™ and its affiliates to collect, analyze, and store my data for research and educational purposes, in compliance with applicable privacy regulations.
  • My information will not be shared with any third parties without my explicit consent, except as required by law.

 

I have read and fully understand the information provided above. I consent to the BioMetrixTM BioScan process and acknowledge that this process is for research purposes and that I am not receiving medical advice, diagnosis, or treatment. I agree to be addressed and treated as a client.
